Thank you for choosing Sunnybrook Dental for your dental needs.

We provide a welcoming environment, where you'll feel confident in the high level of dental care you receive. We will send you our New Patient paperwork before your first visit, or you can find them. Feel free to fill this out beforehand and bring with you, in order to save yourself time at our office. If you can't complete this ahead of time, no big deal. We ask that you then please arrive 10-15 minutes early, and we'll provide you with the paperwork in office. Our New Patient paperwork acquaints us with you, your dental and health history, and any concerns you may have. We also gather the necessary consent forms for treatment and information on your dental benefits, if we'll be submitting claims for you on your behalf.

Dental Benefits

Sunnybrook Dental Benefits

Our office is committed to helping you maximize your insurance benefits. As a service to our patients, we will bill your dental benefit companies for services.

Because dental benefits vary with each policies specific contract, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of these contracts. Your patient portion must be paid per the contract agreement. Our estimated portion will be due on date of service. We will then submit your claim to your benefit provider on your behalf and when we receive your explanation of benefits, we will let you know of any remaining portion, or credit, per the contract." ; third para - change to this "If you have any questions, our courteous staff is always available to answer them. Dental benefit policies vary, so we will review your specific plan to determine the appropriate course of action. If we're unable to answer your question, your dental benefit provider is always the primary source for answering specific questions in regards to your individual contract for coverage.

As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental benefits. Insurance policies vary, so we will review your insurance to determine the appropriate course of action.

Once treatment has started, we will file your claims. To help us with your dental benefit filing, please complete the dental benefit questions on our New Patient Registration Forms. Sunnybrook Dental accepts most dental benefit plans. Please have your dental benefit information accessible when scheduling your first appointment - that way we can verify your dental benefits and minimize your out-of-pocket expense!
